Gold Level Long-Term Care Facility Honor Roll Awardees:
Action: Benedictine Living Community Dickinson is meeting with the resident council and using pamphlets to review appropriate antibiotic use with families and residents. They are also partnering with their pharmacist to complete antibiotic time outs and de-escalation of antibiotics.
Collaboration: This facility is partnering with their local hospital to share an antibiogram and work on IV to oral de-escalation prior to admission into the nursing home. They are also partnering with dental offices to ensure appropriate antibiotic use prior to appointments.
Action: Missouri Slope uses care conferences, one on one sessions, and provides handouts for residents and families on infectious diseases and antibiotic stewardship. They also have started having their pharmacist initiate recommendations for antibiotics prior to dental appointments and use antibiotic time outs.
Collaboration: This facility has implemented duration limits for antimicrobial treatment of uncomplicated cystitis within its facility. It provides feedback to all physicians that write for antibiotics outside of their policy, are tracking data on resistance due to urinary pathogens, and sends letters to all physicians (including local clinics and ERs) the results of this policy.
Action: Northwood Deaconess Health Center provides handouts on antibiotic use, resistances, and a variety of infectious diseases. They have issued the Sanford Antimicrobial Therapy guide to providers to follow prescribing recommendations and are working on using urine culture evidence for UTI treatment while avoiding fluoroquinolones to reduce C. Diff risk.
Collaboration: The facility has worked with the local hospital to be included on the transition of care teams daily multidisciplinary rounds to help support admissions into the facility.
Silver Level Long-Term Care Facility Honor Roll Awardees:
Action: Aneta Parkview Healthcare Center attends resident council meetings to educate residents on antimicrobial stewardship using AHRQ provided toolkits and the “Be smart about antibiotics” handout. They also have implemented SBARs for all suspected infections.
Action: Elm Crest Manor provides education to families and power of attorneys via a yearly letter describing antimicrobial stewardship, how antibiotics work, and when they should be prescribed. They also review all antibiotic starts and provide feedback if criteria for antibiotics were met or not.
Action: Good Samaritan Lakota’s administrator educates all staff and residents during “Stand Up” each morning on best practices for infection control as well reminds staff of the “Tag of the Week” which includes antimicrobial stewardship topics. They also have created several forms to help communication between nursing and physicians when infections are suspected.
Action: St. Vincent’s Care Center is educating residents and family members on antibiotics with printed materials and discussing stewardship during care conferences. They are also preforming 48-hour antibiotic time outs to check for appropriate antibiotic use and reviewing indications for specimen collections.
