Every quarter, the North Dakota Department of Health and Human Services (NDHHS) Immunization Unit assesses provider-level coverage rates for infants, children, adolescents, and adults for North Dakota immunization providers enrolled the Vaccines For Children (VFC) Program and North Dakota immunization providers that serve the state's adult population. Providers who have met or exceeded the immunizations goals for the quarter will be recognized on the immunization provider honor roll. Provider-level coverage is also assessed in July for influenza vaccination coverage for the complete flu season. Providers that meet the immunization goal for influenza vaccination are also recognized on the immunization provider honor roll.
Infant Immunization Honor Roll
The Immunization Unit would like to recognize those providers who have met or exceeded the goal of 80% immunization coverage for the 4:3:1:U:U:1:U (4 DTaP, 3 polio, 1 MMR, hepatitis B up-to-date, Hib up-to-date, 1 varicella, PCV up-to-date) vaccine series for infants 24 - 35 months of age up-to-date by 24 months.

Pediatric Immunization Honor Roll
The Immunization Unit would like to recognize those providers who have met or exceeded the goal of 95% immunization coverage for 5 doses of DTaP, 3 doses of hepatitis B, 2 doses of MMR, 4 doses of polio and 2 doses of varicella vaccine for kids 6 years of age entering kindergarten.

Adolescent Immunization Honor Roll
The Immunization Unit would like to recognize those providers who have met or exceeded the goal of 80% immunization coverage for one dose each of Tdap and meningococcal conjugate (MCV4) vaccines, 80% of males and females up-to-date with the complete HPV vaccine series and 90% for two doses of varicella vaccine for adolescents 13 - 15 years of age.

Adult Immunization Honor Roll
The Immunization Unit would like to recognize those providers who have met or exceeded the goal of 90% immunization coverage for adults 65 years of age or older with one dose of pneumococcal conjugate (PCV) or pneumococcal polysaccharide (PPSV23) vaccine administered after age 65.

Influenza Immunization Honor Roll
The Immunization Unit would like to recognize those providers who have met or exceeded the goal of 70% immunization coverage for one dose of influenza vaccine for at least one age group during the influenza season. The age groups include infants 6 months - 4 years, children 5 - 12 years, adolescents 13 - 17 years, and adults 18 - 49 years, 50 - 64 years and 65 years of age and older.
