If you wish to receive immunization reminder and recall communication electronically (via text or email), please complete the Electronic Notification Opt-In Form.
If you no longer wish to receive any reminder and recall communication from the Immunization Unit, please complete the Recall Exclusion Form.
Infant Recall
Four times per year, the immunization unit sends letters to the parents of infants 19-35 months of age who are behind on their routinely recommended immunizations, based on the NDIIS.
School Reminders
Prior to the start of each school year, the immunization unit sends letters to parents of children entering kindergarten, 7th, 11th, and 12th grades who have not received all of the immunizations their child needs in order to attend school in North Dakota.
Adolescent Recall
Four times per year, the immunization unit sends letters to parents of children 12-17 years of age who are behind on their routinely recommended immunizations, based on the NDIIS.
HPV Reminders
Four times per year, the immunization unit sends postcards to parents of children who have recently turned 11 years old reminding them of the importance of getting their child vaccinated against human papillomovirus (HPV).
Influenza Reminders
During the annual respiratory season, the immunization unit sends reminder letters to parents of children 6 months through 17 years who need a dose of influenza vaccine for the current season. Additionally, the immunization unit sends letters to parents of children 6 months through 8 years of age who need to receive a second dose of influenza vaccine for the current season.
Adult Recall
Periodically, the immunization unit sends postcards or letters to adults 50 years of age or older who are not up-to-date with their routinely recommended zoster and/or pneumococcal immunizations.
