Children with Special Health Care Needs Systems Development/Medical Home Projects

Funding for innovative projects that work towards supporting and improving the overall health and well-being of CSHCN. Projects may focus on care coordination, collaboration, medical home, early and continuous screening and surveillance, services and supports for CSHCN or any other area that improves health outcomes of CSHCN.
