It is possible to live at home longer with home and community-based services and support for older adults and individuals with physical disabilities.

An Assisted Living Facility is a building that has at least five (5) separate living units where individualized support services accommodate an individual’s needs and abilities to maintain as much independence as possible. 

This assistance program helps qualifying individuals pay for the cost for care in a licensed basic care facility. Individuals pay only part of the cost. 

Provides support and services to unpaid caregivers (family members and friends) so they can continue to care for older individuals and others with physical disabilities who need help with everyday activities.

QSPs are individuals or agencies that provide services to older adults and adults with physical disabilities who receive in-home and community-based services funded by HHS.

Advocates for the health, safety, welfare and rights of residents of long-term care facilities, including nursing homes, assisted living, basic care, and swing bed facilities and helps residents and their families resolve concerns. 

North Dakota and the USDOJ reached a settlement agreement to expand access to in-home and community-based care services that enable people with physical disabilities to live in the community with support. In-home services can usually be provided at a lower cost than institutional care.

Together with partners and individuals, HHS is developing a 10-year, strategic Multigenerational Plan for Aging to help transform and improve coordination of programs and services to improve the health and well-being of North Dakotans as they age. 

Helps Medicaid members with disabilities transition/move from qualified care facilities (nursing homes, Life Skills and Transition Center) to homes and apartments in the community. Working with community partners, MFP connects people to housing, services and support.
