Medicaid was authorized in 1966 for the purpose of providing an effective base upon which to provide comprehensive and uniform medical services that enable persons previously limited by their circumstances to receive needed medical care. It is within this broad concept that the Medicaid Program in North Dakota participates with the medical community in attempting to strengthen existing medical services in the state.
Funding is shared by federal and state governments, with eligibility determined at the county level. Medicaid pays for health services for qualifying families with children, and people who are pregnant, elderly, or disabled.

Requesting Form 1095-B, Health Coverage (Posted Jan. 17, 2023)
Form 1095-B is a tax form that reports the type of health care coverage or health insurance each individual in your household had for the previous year. For the 2022 tax year, the IRS determined that individuals do not need this information to complete and file their tax returns.
North Dakota Health and Human Services will not be mailing 1095-B forms to Medicaid members. Copies are available starting Feb. 21, 2023 upon request through one of the following options:
- Call the Medicaid Eligibility Call Center at 1-844-854-4825,
- Email your request to medicaidcc@nd.gov
- Send a written request to the following address: North Dakota Health and Human Services, Medicaid Eligibility, 600 E. Blvd. Ave., Bismarck, N.D. 58505
Reporting Medicaid Fraud
To report suspected Medicaid Fraud, email medicaidfraud@nd.gov, or call 1-800-755-2604 - select option 8 to speak with an attendant. You may also complete the Surveillance and Utilization Review Section (SURS) Referral form and submit it to the Department.
