All Of Us In Recovery offers comprehensive Case Management that provides client support in accessing services in the community, such as behavioral healthcare, peer support, housing, medical care, advocacy, education, employment, and social and spiritual opportunities to improve their lives in recovery.

Community Options provides evidence-based, person-centered services to individuals and families residing in North Dakota communities.

Care coordinators will:

  • Connect you with peer support.
  • Assist you in finding and retaining employment.
  • Assist you in your education goals.
  • Assist you in identifying barriers and how to overcome barriers by making appropriate referrals.
  • Assist in finding housing.

F5 Project

F5 Project provides transitional and re-entry support for those who were and are incarcerated before they are released. We pride ourselves on working on the inside to help bridge the community. We believe in meeting the individual where they are at, and that sets us apart from other organizations in the same sector. We believe that meeting the individual where they are at will help with character development, parenting, employment readiness, and social engagement. 

We believe that not only is the person being transformed, but our community as well. That is why we have community meetings where our staff, the people coming out of incarceration and the community can engage one another and destroy stigmas. These connections heal worldviews, families, and communities. As part of the transition and re-entry support, we provide safe and affordable housing with peer mentorship. The peer will help with employment support, positive social networks, and transportation. A peer is defined as someone who was formerly incarcerated that has lived in the shoes of the person they are providing support for.

F5 Project works with the state of North Dakota on the Free Through Recovery program. Free Through Recovery (FTR) is a community-based behavioral health program designed to increase access to recovery support services for individuals engaged with the criminal justice system who have serious behavioral health concerns. Free Through Recovery is a partnership between the Department of Human Services and the Department of Corrections and Rehabilitation. The mission of Free Through Recovery is to improve healthcare outcomes and reduce recidivism by delivering high-quality community behavioral health services linked with effective community supervision. The goals are to improve engagement in quality services and to provide access to individualized services that are responsive to each person’s specific needs.

F5 Project is proud to be joining the state of North Dakota and the North Dakota Department of Behavioral Health’s newest program, Community Connect. Community Connect helps pregnant women, families, caregivers, and individuals by providing a one-stop entry point for needed services, such as housing, employment, financial, parenting, physical health, and community connections.

F5 Project has homes in Fargo, Grand Forks, Bismarck and Moorhead, MN. We offer care coordination for Free Through Recovery (FTR) and Community Connect and peer support services in Fargo, Grand Forks, Bismarck, Jamestown, Dickinson, Minot, and Williston.

Flourishing Growth

Flourishing Growth is a new agency with Community Connect and Free Through Recovery. I do have experience and many challenges in my lifetime. I would like to take the chance on helping participants with their journey to develop into their own individual person. As a Care Coordinator, I will be working side-by-side with Human Services Center, PO, LED, Counselors, Re-entry, Peer Support.  

I can help with:  

  • Assistance with housing  
  • Assistance with employment and financial services  
  • Advocacy  
  • Working on goals which is a big thing. 
  • Helping out with paperwork for assistance programs if need 
  •  Assistance for your physical health and mental health needs and referrals  

As my motto says, “No one left behind”. I'm committed to my participants even if they are not committed to me in the beginning. Let's learn together to take the steps of success. To change the communities, look for recovery. I hope that down the road they reach out even when they don’t need the support. To insure me their recovery is still going strong. Even if there are bumps in the road. They allow me to help them pick up the pieces.  As my motto says, “No one left behind”. Middle of weeks are hard, come join me Wednesday nights for bible study and games and additional speakers.

Independence, Inc. (Provider not currently accepting new participants at this time)

Independence, Inc. is a resource center for independent living. Our agency is peer-driven and committed to maximizing the independence of people in recovery throughout the northwestern region. We provide a variety of services designed to empower and remove obstacles preventing individuals from fully participating in society. Services may include one-on-one skills training or assistance in obtaining access to housing, employment, transportation, education and financial independence.

Living Free (Provider not currently accepting new participants at this time)

We have all made and continue to make mistakes. Sometimes we struggle with making the same mistakes over and over which leads to relationship problems with our parents, siblings, children, and significant others. We should all have opportunities to learn and move on from our mistakes. Living Free offers choices, hope, and support to help to live a life of freedom from mistakes and wrong choices. Living Free plans to make the needed referrals for treatment and mental health issues, which is so common in today's society. Living Free will work with participants on stable housing and employment. Living Free will support participants to become free of drug addictions, alcohol addictions, gambling addictions, and mental health concerns. Living Free will assist participants in becoming stable in the community and support participants in continuing to live a free lifestyle, independent of dependency, joblessness, instability in housing, and unhealthy relationships.

Revive (Aabiziishin) LLC

Revive (Aabizhiishin: s/he comes back to life; s/he revives) is an independently owned provider focused on recovery from addiction, managing mental health along with support for those with PTSD/ generational trauma. Revive provides guidance for individuals with physiological and safety needs to jump start a new way of life. Revive will assist in finding and retaining employment; educational goals; finding housing and identifying obstacles and how to overcome obstacles in day-to-day life.

Seasons in Life

Seasons in Life understands the challenges individuals may face during difficult circumstances in life, crisis or recovery. Using years of devoted experience, I am committed to helping participants with each step of their journey. Providing individualized attention from a dedicated care coordinator I can assist with the following:

  • Assistance securing adequate housing
  • Exploring employment opportunities
  • Advocacy
  • Help and support completing required paperwork for community and state assistance programs
  • Assist participants with health and/or mental health referrals
  • Connection with peer support
  • Assist with many other numerous tasks that are crucial in living a healthy lifestyle

Just as seasons change from cold and depressing to warm and bright, life has its seasons, I am here to provide assistance in guiding participants through the changes in their lives in order to keep moving forward. Seasons In Life is a dedicated program that takes a personalized approach and strives for each participant’s well-being and success.
