Eligibility Requirements:
- Licensed health care professionals practicing in North Dakota
- Willingness to commit to a specified service obligation

Application Process:
- Complete the online application form on the PCO website.
- Gather the following documents:
- Current resume or curriculum vitae (CV)
- Signed employment contract with addendums (if applicable)
- Copy of North Dakota provider license
- History of medical licenses held
- Description of any litigation you are party to
- Current loan statement from each lender (consolidated into one PDF)
- Three letters of recommendation, including one from a direct supervisor and one from another organization official
- The applicant’s letter of support should clearly and concisely address the following:
- Please expand upon your relationship with the applicant.
- The reference should describe how the applicant performs as a member of a team and discuss the applicant’s experience with teamwork and openness to working with others.
- The reference should describe how the applicant demonstrates commitment to the delivery of high-quality health care to medically underserved patients. This can include, but is not limited to, the applicant’s commitment to rural and/or underserved patient populations, commitment to safety-net healthcare, or commitment to improving the health care of their community.
- Signed statement of endorsement and financial commitment from your employer (ND Healthcare Professional LRP) this is not required for initial application, if you’re selected we will contact you to obtain the form.
- Upload the required documents as attachments on the application portal.
- Await notification regarding the status of your application.
