Men's Health Chronic Disease Management
Chronic diseases are conditions that persist for long periods of time without the ability to be cured. These diseases require varying levels of management throughout one’s lifetime to prevent negative side effects or worsening. The programs and resources below aim to help those living with chronic conditions to manage their own care, or the care of others.
- North Dakota Community Clinical Collaborative (NDC3): Offers programs to help manage chronic health conditions (including high blood pressure heart disease, COPD, arthritis, diabetes and more), prevent falls, and foster wellbeing. Many of the classes are free, and some offer virtual options to enable you to attend from anywhere!
- HIV Care/Ryan White Program: The Ryan White Program Part B provides case management and reimbursement of services for persons living with HIV in North Dakota.
- Heart Disease/Stroke Prevention
- Asthma
- Alzheimer’s Association
- Metabolic conditions/Metabolic Food Program: Provide metabolic formula for free to males with PKU and MSUD until they are 26. After that age, men can purchase from Special Health Services at cost, regardless of income.
