The funding is designed to support practical, locally driven solutions that help rural and tribal communities through four pillars:

New Vendors, Contractors, Providers

If you’re new to doing business with the state, there are a few required steps you’ll need to complete before contracting and receiving payment(s). 

1. Register with the Secretary of State to perform business in the state.

2. If your organization has not been paid by a state entity before, you must register with vendor registry before you can be paid. 

3. ND HHS will require a W9 form to set up by your organization within our contract system. If your organization would like to be paid with electronic funds transfer, you need to submit the substitute IRS form W-9.

4. If your organization will receive funding under a federally funded program, you must register in SAM.gov and obtain a Unique Entity Identifier (UEI) prior to contract execution or payment. Your SAM.gov registration must remain active for the duration of the award.

  1. Can year one funding be used for planning and developing a new rotation program coordinating nursing internships across a statewide network of rural clinics, while the actual implementation wouldn’t occur until after year one? It would likely take a full year to develop the plan and infrastructure.
    Answer: Yes, year one funding can be used for planning and developing a new rotation program.
     
  2. Can a single entity submit two separate proposals for this funding?
    Answer: No, an organization should submit one proposal in this first year. If there are multiple strategies and activities, please number in order of priority.
     
  3. I represent two different organizations who both want to apply. Is there an easy way for me to accomplish this in Qualtrics? When I click on the hyperlink, it directs me to the first application I started. How can I start on another application for a different organization?
    Answer: There are a couple of options for submitting applications for two different organizations. First make sure you complete and submit the first application before starting the second one, you could use a different browser or a different computer.
     
  4. Are all the grants in the category, "strengthen and stabilize rural health workforce" released?
    Answer: No, not all funding opportunities in Initiative 2 have been released. At this stage, all funding opportunity details have not been finalized. We encourage you to visit our website and sign up for RHTP updates to receive timely notifications as new opportunities and resources are released.
     
  5. We submitted an application for a funding opportunity and now recognize that some of our proposal aligns with a new funding opportunity just posted. Do we need to apply for both? If we apply for both, can we be awarded for both?
    Answer: Yes, applicants must submit an application for each funding opportunity that they want to be considered for. Applicants can be awarded multiple funding opportunities. However, the same project cannot be funded by two different funding opportunities.
     
  6. If we have one EIN that includes all facility locations across the state and different locations have different projects, they want to apply for can each location submit an application while using the same EIN?
    Answer: Facilities with the same EIN can apply separately if they are in separate locations. This should be clearly noted in the application.
     
  7. Is there a limit on how small or large an agency could be? 
    Answer: No.
     
  8. Is there a minimum requirement for the quantity of the target population?
    Answer: No, there is not a minimum requirement on quantity of the target population.
     
  9. Is there a list of new learner types? Please give examples if you can.
    Answer: New learner types would be any new rural rotation student you do not currently have; this may look different for each applicant.
     
  10. Can a remote employee who works with and benefits rural patients clinical training experience, such as telehealth professionals?
    Answer: No, this funding opportunity is to embed new learner types into rural communities.
     
  11. Can a contracted staff member participate?
    Answer: No, contracted or locum tenens providers are not eligible, as these efforts are intended to focus on developing a long-term provider base.
     
  12. If an organization does not subject physicians to non-compete clauses are salaries an allowable cost? Are there any other salaries that are an allowable cost?
    Answer: We need more clarification on the non-compete clause, however salaries may be an allowable cost ensuring they align within the proposed scope, are not duplicated and do not exceed RHTP’s salary rate limit.
     
  13. Would funding cover planning, development and implementation for a new student rotation program?
    Answer: Yes, this funding opportunity includes planning, development and implementation for a new student rural rotation program.
     
  14. At the very top of the application, where you ask for an address, do you want the address tied to the EIN or the address where the project work happens?
    Answer: For the question at the top of the application, use the address where the project will occur and for the budget use the address tied to the EIN.
     
  15. If we have a comprehensive idea and that idea that can fit under multiple subcategories (eg. one idea that encompasses Rural Healthcare Rotations and Train in Place and more under the goal Strengthen and Stabilize Rural Health Workforce), can we apply for one grant or do we have to split up/piecemeal the idea into the multiple proposed grant categories. And then if we split the project into five grant categories, would we then have to do reporting/metrics for five different grants?
    Answer: You will need to apply for each grant separately as each grant has different criteria.
     
  16. In the budget form, you must select an RHTP Priority Area. Is this opportunity considered to expand rural healthcare training pipelines?
    Answer: Yes, this funding opportunity is “expand rural healthcare training pipeline.”
     
  17. Would salaries fall under the administrative restrictions of 10%?
    Answer: This would depend on the personnel involved and if they are directly serving to accomplish the scope of work or administration. Salaries may also be subject to RHTP’s salary rate limit.
     
  18. Can you speak more about the five-year service commitment if we offer housing support?
    Answer: https://www.cms.gov/files/document/5-year-service-commitment-fact-sheet.pdf. North Dakota is following CMS’s requirements for the five-year service commitment.
     
  19. When you say your team is available to help, who would we be our contact? 
    Answer: During the application period, the best way to get assistance is through the Rural Health Transformation Frequently Asked Questions. Once awards have been chosen, you will work directly with the funding manager and team for this specific opportunity.
     
  20. Our entity would host the students, could we offer a stipend to the students to incentivize them to come to our rural location sites?
    Answer: A stipend to the students to incentivize them to come to the rural location could be allowable, as a reminder, individual awards may lead to a five-year service commitment.
     
  21. Is there any flexibility in the 50% rural requirement, if the organization can make a case that the training is still needed related to rural patients, Native American patients or competition needs compared to larger entities?
    Answer: There is no flexibility in the 50% rural requirement.
     
  22. Question about eligibility, specifically mentioned tribes, but excluding IHS, can someone expand more on this?
    Answer: Per CMS’ requirements, federally operated IHS facilities are not permitted to be subawardees or contractors with RHTP funds. Tribal operated IHS facilities may be subawardees of RHTP funds.
     
  23. Can we bring learners into an urban area for training or must the training stay within the rural community?
    Answer: This funding opportunity is for embedding new learner types into rural communities, so training rotations must be done in rural or tribal communities, if the rural rotations include some training in urban areas details of why this training needs to be completed in urban areas must be included in the application.
     
  24. Would a paid summer internship vs. a rotation be feasible if it would bring in a new learner type to our facility?
    Answer: These funds are for expanding rural rotations not internships. However, depending on the scope, this may be an allowable project. Additionally, individual awards may lead to a five-year service commitment.

  1. With such a tight turn-around for this competition, how is it feasible to create a well developed proposal?
    Answer: We appreciate you raising this concern. North Dakota Health and Human Services (ND HHS) will continue to monitor its timelines to ensure RHTP grant fundings opportunities are timely posted, evaluated, and awarded within the tight timeline established by the Centers for Medicare and Medicaid Services (CMS). 
     
  2. Are all of the grants in the category, "strengthen and stabilize rural health workforce" released? This grant doesn't appear to relate to new to the field pipeline, will there be another grant to cover the "expand rural healthcare training pipeline" strategic priority?
    Answer: No, not all funding opportunities in initiative 2 have been released. At this stage, all funding opportunity details have not been finalized. We encourage you to visit our website and sign up for RHTP updates to receive timely notifications as new opportunities and resources are released.
     
  3. In the RFP, in the funding opportunity section second bullet point it, second sentence states, "This must include training on coding and billing." Is this a must or is it suppose to say can include training on billing and coding?
    Answer: Business-practice training that promotes the sustainable employment of staff receiving training in place must include training on coding and billing. This was noted in ND’s RHTP application, as noted on pages 4-6 of the appendices.
     
  4. We submitted an application for a funding opportunity, and now recognize that some of our proposal aligns with a new funding opportunity just posted. Do we need to apply for both? If we apply for both, can we be awarded for both?
    Answer: Yes, applicants must submit an application for each funding opportunity that they want to be considered for. Applicants can be awarded multiple funding opportunities. However, the same project cannot be funded by two different funding opportunities.
     
  5. If we want to submit more than one application for this opportunity, how do we do that? When I click on the Qualtrics link in the RFP, it takes me to the application I've already started.
    Answer: One application per organization should be submitted. As stated in the application guidance, if there are multiple strategies, please number in order of priority.
     
  6. Can a higher education institution propose a program for individual skill building awards by itself, or must it partner with a health care employer who provides the awards to its employees? If it can propose a program by itself, would the workers doing the program then have a five-year commitment to their respective employer?
    Answer: Yes, however, the five-year service commitment must be managed by the awarded applicant. ND HHS will have an agreement with the awarded facility. The awarded facility must manage the five-year service commitment. As noted in the guidance, formal copies of policies, procedures, and service commitment agreements will be submitted to ND HHS for review and approval before any financial incentive can be issued to an employee and before submission of a request for reimbursement. Instructions on how to submit these will be included in the agreement. Additionally noted in the guidance, staff completing a training program must be employed and on the payroll at the time this funding application was posted. If funds are used for staff to complete a training program, please identify staff person in the budget.
     
  7. One section of the RFP says the plan “must” include training on billing & coding, while a different section in the RFP says “can” including training on billing and coding. Can you please clarify if training on billing and coding is a required activity for this funding? What if the billing and coding staff are all housed at the urban headquarters, while the project being submitted is specifically for a rural site of the organization?
    Answer: Coding and billing must be included, as noted on pages 3 of the application guidance. Page 9 of the guidance states the definition of train in place, noting this may include business practice training on coding, billing and more that promotes the sustainable employment of staff receiving training or credentialing in place. The may is stated here as applicants do not have to include business practice training in their proposal.
     
  8. If we submit a proposal for a “planning” of a train in place program, but it doesn’t take the full grant year to plan, can we shift to implementation and start training people during the year one grant?
    Answer: An awarded applicant could shift to implementation. However, funds for this train in place funding opportunity are only for the planning phase. ND HHS anticipates additional funding opportunities in future RHTP years for implementation.
     
  9. Regarding the five-year service commitment for the workforce funding: Are there any places where North Dakota’s standards are different or more specific than what is outlined in this CMS document? https://www.cms.gov/files/document/5-year-service-commitment-fact-sheet.pdf
    Answer: North Dakota is following CMS’s requirements for the five-year service commitment.
     
  10. Regarding the five-year service commitment for the workforce funding opportunities: How will this be tracked, reported and enforced? Will the state have pay-back penalties for those who don't complete the five-year term? Do commitments apply to a specific facility, or to ND as a whole?
    Answer: ND HHS will have an agreement with the awarded facility. The awarded facility will have a relationship with the employee, and the service commitment must be managed at that level. The facility must determine their action in relation to the employee and include this in their policies and procedures. There is a sample service commitment agreement on the funding opportunity webpage.
     
  11. Does an educational organizations need to apply with a rural community healthcare organization? Can an educational organization apply on there own to offer virtual/remote training to rural residents across the state? 
    Answer: Yes, however, the training must be provided in a rural community and be sustainable in the rural areas. The applicant must provide justification on how training can be provided and sustained in rural areas. Additionally, if individual skill-building awards are offered, the awarded applicant must manage the five-year service commitment and staff completing a training program must be employed and on the payroll at the time this funding application was posted and be identified in the submitted budget.
     
  12. If an entity has both urban and rural locations, are applications accepted for just the rural location?
    Answer: The training for the train in place funding opportunity must occur in the rural area. Additionally, for RHTP funding opportunities, the entities within the cities of Grand Forks, Fargo, West Fargo and Bismarck are considered urban and do not qualify for RHTP funding opportunities unless the population served by the grant applicant is at least 50% ND rural citizens or the focus of the grant funding will be used for ND rural citizens. RHTP funding must be used to support ND rural citizens.
    If the applicant is urban, they must justify how the training would be provided in a rural area and serve at least 50% ND rural citizens.
     
  13. If we, as an educational organization, are submitting an application for the train in place grant and a nonprofit organization asks to list us as a partner in their application, may we allow them to list us?   
    Answer: RHTP cannot duplicate pay for services rendered and would require cross examination to ensure there is not crossover in the scopes of work.
     
  14. Some of the state's language regarding the 50% threshold say "50% rural or tribal," and other state language says "50% rural and tribal."  How should we interpret that? If a project is focused on Native American populations within an urban area, does that meet the requirement?  
    Answer: For RHTP funding opportunities, the entities within the cities of Grand Forks, Fargo, West Fargo and Bismarck are considered urban and do not qualify for RHTP funding opportunities unless the population served by the grant applicant is at least 50% ND rural citizens or the focus of the grant funding will be used for ND rural citizens. RHTP funding must be used to support ND rural citizens. If the applicant is urban, they must justify how the training would be provided in a rural area and serve at least 50% ND rural citizens.
     
  15. Does this one require board approval?
    Answer: No, this will vary by funding opportunity.
     
  16. Can this funding be used to support current staff attending trainings/programs that leads to certifications or degrees? (For example, becoming a certified footcare nurse or tuition reimbursements for moving from an LPN to RN).
    Answer: The train in place funding opportunity allows for the offering for individual skill-building awards. As stated in the guidance, these awards can support the existing workforce in obtaining new credentials, enhancing competencies or completing approved training programs. However, these training opportunities must be provided in the staff’s rural communities and certificate or degree programs would require a five-year service commitment.
     
  17. Will the Qualtrics submission forms for all of the RHTP opportunities have the save and return function?
    Answer: Yes, progress is automatically saved every time you click the next button at the bottom of a page. If you need to stop, you can close your browser and resume within one week. To resume, click the original survey link again from the same device and browser. This feature will not work if you are using a provide or incognito browser window.
     
  18. On the budget drop down menu, what is the priority area for this opportunity?
    Answer: Expand rural healthcare training pipeline.
     
  19. If, for example, you are proposing a physician training in place, do those physicians have to be currently employed with the organization?
    Answer: For applicants proposing individual skill-build awards, the physicians must be currently employed. If an applicant is proposing to use the funds to plan and develop a train in place program, the physicians do not need to currently be employed at this time; as a reminder the planning option does not cover implementation.
     
  20. Can scholarships be offered by healthcare institutions for staff to attend higher ed institutions to receive the certification?
    Answer: The training must be provided in a rural community and be sustainable in the rural areas. The applicant must provide justification on how training can be provided and sustained in rural areas. Additionally, individual skill-building awards may lead to a five-year service commitment. CMS has indicated that ND HHS and award recipients should not use the term “scholarship” and instead use a different term, such as education award, awards, tuition waiver, or funding awards.
     
  21. Can this funding be used to offer tuition waivers or scholarships?
    Answer: The training must be provided for the existing workforce in a rural community. The applicant must provide justification on how training can be provided and sustained in rural areas. Additionally, individual skill-building awards may lead to a five-year service commitment. CMS has indicated that ND HHS and award recipients should not use the term “scholarship” and instead use a different term, such as education award, awards, tuition waiver, or funding awards.
     
  22. Are there character or word count limits, for example per question, within the Qualtrics application?  
    Answer: Some fields may be limited. Applicants are encouraged to be as concise and specific as possible in their responses.
     
  23. Is a state agency that functions as a K–12 school eligible to apply, or are state agencies excluded regardless of their educational role?
    Answer: As a reminder, the train in place funding opportunity is for the existing workforce, not students.
     
  24. Does a person getting support for training tuition have to be employed at a healthcare organizations or can these funds be used for scholarships for students attending a virtual/remote program through one of the state's colleges or universities but have not entered the workforce.
    Answer: The train in place funding opportunity is for the existing workforce. As noted in the guidance, staff completing a training program must be employed and on the payroll at the time this funding application was posted. If funds are used for staff to complete a training program, please identify staff person in the budget. CMS has indicated that ND HHS and award recipients should not use the term “scholarship” and instead use a different term, such as education award, awards, tuition waiver or funding awards.
     
  25. For clarification....if you have two proposal ideas for train in place....one that impacts the eastern part of the state and another proposal for the western part of that state, those proposals can't be made separately, you have to include both those proposals in a single, Qualtrics application? And presumably with the budget limit of a single project?
    Answer: Yes, an organization should submit one application. If there are multiple strategies and activities, please number in order of priority.
     
  26. How does ND HHS define and document "ND rural citizens served"?  If our organization is headquartered in an urban excluded city will we qualify if we have satellite office(s) in rural areas; if our patients are rural; staff is working out of a rural office?  How is the 50% rural populations served measured - patients, encounters, census?
    Answer: An urban applicant must provide justification for how the population served by the applicant is at least 50% ND rural citizens or the focus of the grant funding will be used for ND rural citizens. Rural citizens may include patients, workforce or other.
     
  27. So if someone goes thru a certificate program we offer through this funding does that person have to abide by a five-year service commitment to our organization. Is this the intent?
    Answer: Yes, individual skill-building awards to obtain a certificate will require a five-year service commitment. The current funding for planning and development of a train in place program does not include implementation and would not involve a five-year service commitment.
     
  28. When referring to "individual skill-building awards," does that mean that funding can be used as scholarships to external training opportunities that are virtual or within the rural community (meaning the training would not be organized or administered in-house by the applicant)?
    Answer: Individual skill-building awards must be offered and completed in the rural community. This can include offering courses virtually. As a reminder, individual skill-building awards may lead to a five-year service commitment and staff completing a training program must be employed and on the payroll at the time this funding application was posted. If funds are used for staff to complete a training program, please identify staff person in the budget. CMS has indicated that ND HHS and award recipients should not use the term “scholarship” and instead use a different term, such as education award, awards, tuition waiver or funding awards.
     
  29. If the training program takes place online and is targeted to rural ND, does that qualify for this grant?
    Answer: Individual skill-building awards must be offered and completed in the rural community. This can include offering courses virtually. As a reminder, individual skill-building awards may lead to a five-year service commitment and staff completing a training program must be employed and on the payroll at the time this funding application was posted.   30. One of the metrics is that 200 students will complete train in place programs - if year one funding is for planning and setting up infrastructure, is the expectation that 200 students will complete program years 2-5? 
    Answer: Yes. In this instance, student refers to those completing a train in place program, who must be part of the existing workforce.
     
  31. If an employee virtually attends a university in an urban area; however their virtual location is rural could the entity that applies for this funding provide tuition reimbursement for the employee.  
    Answer: Individual skill-building awards must be offered and completed in the rural community. This can include offering courses virtually. As a reminder, individual skill-building awards may lead to a five-year service commitment and staff completing a training program must be employed and on the payroll at the time this funding application was posted. If funds are used for staff to complete a training program, please identify staff person in the budget.
     
  32. Just to clarify, only 15 awards will be awarded for approximately $135,000 per award, correct?
    Answer: Yes, approximately 15 awards will be awarded for approximately $135,000 per award.
     
  33. Are rural school educators who wish to become certified as school or clinical counselors or social workers within the rural school, eligible to receive training?  
    Answer: The train in place funding opportunity is intended to support the rural healthcare workforce. At this time education staff are not included.
     
  34. Does all training need to take place in ND? Some programs are not offered in our state.
    Answer: Yes, the train in place programs must take place in rural ND communities. For individual skill-building awards, this can take place virtually.
     
  35. Are organizations able to utilize the funds for training that supports retention practices (e.g., career/clinical ladder)? Additionally, may we utilize the funding for workflow redesign?
    Answer: No, retention and workflow redesign are not eligible for the train in place funding opportunity. They may fall under a different aspect of North Dakota’s RHTP and additional funding opportunities will continue to be posted to address the projects identified in North Dakota’s application.
     
  36. Can we use these funds to pay for current workforce to complete online trainings to obtain certifications?
    Answer: Individual skill-building awards must be offered and completed in the rural community. This can include offering courses virtually. As a reminder, individual skill-building awards resulting in obtaining certifications will require a five-year service commitment and staff completing a training program must be employed and on the payroll at the time this funding application was posted. If funds are used for staff to complete a training program, please identify staff person in the budget.
     
  37. Are you able to provide some clarification and examples on "strong measurable outcomes"?
    Answer: When writing outcomes, applicants are encouraged to use the SMART acronym, which stands for specific, measurable, achievable, realistic and time-bound.
     
  38. If these awards are for year one funding, do you anticipate new applications being accepted for subsequent years or will funding for years 2-5 be following these same awardees for year one?  
    Answer: Yes, we anticipate future funding opportunities for train in place programs.
     
  39. If our proposal needs adjustments for alignment, what level of post-award negotiation is realistic - given ND HHS states the agreement terms will not change?
    Answer: For pre-award budget adjustments, ND HHS may work with applicants on follow up questions or renegotiations before submitting to CMS. Once budgets are submitted to CMS for approval, we may receive additional questions to be addressed with applicants. Post-award budget adjustments may be considered and would require ND HHS and CMS approval before those are made. For the agreement, there will not be any opportunities for negotiation due to the strict timeline of the program.
     
  40. Would clinical counselors working in rural schools be considered healthcare workers?
    Answer: The train in place funding opportunity is intended to support the rural healthcare workforce. At this time education staff are not included.
     
  41. If a local ND resident applied (our provider or nurse) but their online program is offered from a different state (online classes), would that still be an acceptable?
    Answer:  Applicants must be an organization, not an individual. Individual skill-building awards offered from an awarded applicant must be completed in the rural community. This can include offering courses virtually. As a reminder, individual skill-building awards may lead to a five-year service commitment and staff completing a training program must be employed and on the payroll at the time this funding application was posted. If funds are used for staff to complete a training program, please identify staff person in the budget.
     
  42. We have several facility locations across the state. Different locations are interested in the train in place grant for different projects. Can I submit more than one application?
    Answer: Facilities with the same EIN can apply separately if they are in separate locations. This should be clearly noted in the application.
     
  43. If the funding opportunity for bullet one is to develop a new train in place program in year one and to be implemented in years 2-5, will those that received planning money be a priority for funds for implementation or with they have to reapply for funds to implement the training program?
    Answer: ND HHS anticipates additional funding opportunities in future RHTP years for implementation.
     
  44. Does the individual skills building require the individual to use a program within North Dakota or can it use an out-of-state university or credentialing program?
    Answer: Individual skill-building awards can be completed through any appropriate program. As a reminder, individual skill-building awards may lead to a five-year service commitment and staff completing a training program must be employed and on the payroll at the time this funding application was posted. If funds are used for staff to complete a training program, please identify staff person in the budget.
     
  45. Question: The purpose of this funding opportunity is for planning and infrastructure for train in place programs. If we get through the planning process and the infrastructure, can we start operating the program before year two of RHTP funding is announced?  IE – We have a plan in place, we get through the planning phase by October 2027, can we start operating at that point?
    Answer: An awarded applicant could shift to implementation. However, funds for this train in place funding opportunity are only for the planning phase. ND HHS anticipates additional funding opportunities in future RHTP years for implementation.
     
  46. If the cost of equipment needed for a program exceeds $135,000, can we supplement costs with other funding?
    Answer: Yes, there is a box on the budget form titled “Match” that you will list the extra funds you will be providing and can list where they will be coming from in the justification directly below. This box is titled “Source of match funds if applicable”.

  1. Is this not allowed for K-12 students?
    Answer: This program is for students in middle, junior and high school.
     
  2. Does the data tracking stay local or is it saved/used at some higher level outside of the school?
    Answer: The data tracking would stay local to the school, but you would need to share de-identified/aggregate data.
     
  3. Could this funding be utilized by a homeschool group to rent gym space or pay memberships for homeschool students?
    Answer: The funding must be used for the “Zero-Hour” PE initiative, a before-school fitness program and must contain the seven components listed in the funding opportunity. Funding eligibility is restricted to North Dakota’s rural and tribal, public and private schools, regional education associations and/or school consortiums.
     
  4. If we are not structured as a “middle school” can we still include 6th grade students?
    Answer: Yes, if you are not structured as a "middle school,” you can still include 6th grade students.
     
  5. If NDSU partnered with three different school districts, could we request up to $210K (up to $70K per district?)
    Answer: Funding eligibility is restricted to North Dakota’s rural and tribal, public and private schools, regional education associations and/or school consortiums. If a school district says we want to partner with NDSH, the funds would go directly to the school district and the school district would pay NDSH as a subcontractor under their budget template. 
     
  6. Can the application be printed out to be worked on and then information added into Qualtrics for actual application?
    Answer: Yes, the pdf application can be printed out and used when completing the application in Qualtrics. You will use the questions in the background and narrative to assist you in completing the application in Qualtrics.
     
  7. Can a collaborator, grant writer be added to assist with the grant writing process, with the applicant being the one that is responsible for and certifying submission?
    Answer: Yes, however RHTP funds cannot be used to pay pre-award costs.
     
  8. How does funding work when partnering with UND? Does that money go to the district or does that go to UND?
    Answer: This would depend on how you write your application. For example, if a school district says we want to partner with UND to use one of their facilities or something along those lines, it may be possible. The funds would go directly to the school district and the school district would pay UND as a subcontractor under their budget template. Funding eligibility is restricted to North Dakota’s rural and tribal, public and private schools, regional education associations and/or school consortiums.
     
  9. If approved, when can equipment starts to be purchased? And how fast would expenses be reimbursed?
    Answer: Purchasing cannot happen until the contract has officially been executed. So that is not allowable until it has been approved through North Dakota Health and Human Services, CMS and then the contract areas. Typically, expenses will be going through our PRS, which we do have the information on the website on how to get registered for payment request opportunities to be sent. Typically, that'll take less than five business days for processing through the PRS system. And then we also ask for five business days for processing through the AP side, whether it's a check or ACH that you have set up with the state.
     
  10. Hiveclass, a company focused on wellness for K-12 through digital curriculum for PE and mental health. Can we partner with an organization in the state to create a program? We would work with students remotely to provide remote physical education access.
    Answer: No. This program does not fund remote physical education; this program is for in-person programming only.
     
  11. If we wanted to implement something like a HIIT workout program, would we need to develop our own program or show what our program exactly is as part of the application or before receiving funding?
    Answer: You could develop your own program or use an existing program; these details will need to be provided as part of the application. The funding must be used for the “Zero-Hour” PE initiative, a before-school fitness program and must contain the seven components listed in the funding opportunity.
     
  12. If awarded the grant, would it need to be implemented in the fall of 2026 when school starts?
    Answer: You can implement the program at any time however funds must be fully executed by October 30, 2027.
     
  13. If a rural school district is an eligible applicant, can UND College of Education serve as a formal partner, consultant, evaluator, program support, curriculum design, training, evaluation, data management or graduate student instructor support or subcontractor?
    Answer: Yes. Funding can be used to cover the salary of an instructor and/or a consultant if you choose to use them.
     
  14. If UND graduate students help deliver the Zero-Hour PE sessions, should they be listed as consultants, instructors, student workers or subcontracted personnel?
    Answer: UND graduate students could be listed as consultants and/or instructors. Payments for services are allowable.
     
  15. Are stipends or wages allowable for UND graduate students who serve as Zero-Hour PE instructors before school?
    Answer: Yes, if you are not supplanting those wages and make sure you are not replacing this funding with previously agreed funds.
     
  16. Are travel costs allowable for UND graduate students or faculty commuting from UND to a rural school district to deliver or support the program?
    Answer: Yes. Travel costs are allowable costs depending on what the relationship is built into the grant, if they are a subcontractor, that would be a different relationship and typically listed as a fee for service versus if they are listed as a subrecipient to their direct grant and those details would need to be in the proposed budget.
     
  17. Can the budget include mileage reimbursement for UND personnel?
    Answer: Yes. The budget can include mileage reimbursement.
     
  18. Can the budget include compensation for the school PE teacher or staff member if they are doing additional work outside their normal paid duties?
    Answer: Yes. If you are not supplanting funds and make sure you are not replacing this funding with previously agreed upon funds.
     
  19. For a small rural school, is a smaller group of students acceptable, such as 15-30 students, if the program is well designed and measurable?
    Answer: Yes. Small rural schools are eligible for this funding.
     
  20. Can the program target middle school only, high school only or a combined middle/high school group?
    Answer: Yes.
     
  21. Can priority be given to students who are less physically active or not participating in school athletics?
    Answer: Yes
     
  22. Does the program have to meet 2-3 times per week for the full academic year or can it be delivered in shorter cycles, such as fall and spring sessions?
    Answer: The program must meet 2-3 times per week, but does not need to be for the full academic year.
     
  23. What pre/post data are required at minimum? For example, fitness, physical activity, mental well-being, attendance, academic performance or student surveys?
    Answer: You will report on the de-identified data that supports the outcomes you are wanting to achieve.
     
  24. Can UND help with deidentified data collection, analysis and reporting?
    Answer: This would depend on how North Dakota’s rural and tribal, public and private schools, regional education associations and/or school consortiums writes its application. For example, if a school district says we want to partner with UND, it may be possible.   25. If UND wants to use the data for research or publication later, would that require a separate IRB protocol and school approval beyond the grant reporting requirements?
    Answer: This question is outside the scope of the grant funding opportunity.
     
  26. Are student-level academic data allowed for evaluation if they are deidentified or should the project focus only on fitness and wellness outcomes?
    Answer: Yes. Student level data is acceptable if it is de-identified. Academic performance data can be used.
     
  27. Does the application require a letter of support or memorandum of understanding between a rural public school district and UND?
    Answer: A letter of support or memorandum of understanding is not required.
     
  28. Can transportation for students be included if before-school participation creates access barriers?
    Answer: Transportation costs are allowable expenses, but must be specific for the Zero-Hour PE initiative.
     
  29. Can staff be a sub-group?
    Answer: No. This funding opportunity, if specifically for students, may be future funding focused on staff.
     
  30. What sustainability plan is expected after the first funding period ends?

    Answer: Each school within that district could apply for funding. No, they would not risk receiving lower priority. Each school’s application will be reviewed independently.
     
  38. I am wondering if NDSU would be an eligible grant recipient. We are interested in supporting a coalition of schools with implementation

  1. Please describe what you expect a garden layout to look like.
    Answer: The layout of your garden will be specific to what will work best in your community and support you in achieving your outcomes.
  2. Is this opportunity limited to outdoor gardens? Or could an entity invest in an indoor option, such as a hydroponic garden that could be set up inside a school, community center, etc. that could then produce food all year round?
    Answer: Yes, pre-constructed hydroponic garden is allowable as equipment would be considered. However, RHTP funds cannot be used to pay for a foundation as it would be considered construction.
  3. Could a garden be outdoors but have an indoor equipment component so that participants can start the plants indoors and transfer them?
    Answer: Yes.
  4. Would the state consider extending the application deadline.
    Answer: We will not be extending the application deadline, however there will be opportunities to apply in the coming years.
  5. There is a drop-down menu for the RHTP priority area---is the correct response for the garden, Eat Well ND?
    Answer: Yes, the priority area for this funding opportunity is Eat Well ND.
  6. Regarding people component, do you expect proof that different groups would be involved?
    Answer: We do not expect proof of involvement, but would like to understand who will be involved and what role they will play.
  7. Would community flower gardens work with this program? Or does it only cover gardens with fruit and vegetables?
    Answer: Community flower gardens are not eligible for this funding opportunity.
  8. Are hoophouses/high tunnels considered buildings and are therefore not eligible?
    Answer: A hoophouse, high tunnels, greenhouses and pre-constructed garden sheds that are not placed on a foundation are allowable as it would be considered equipment. RHTP funds cannot be used to pay for a foundation as it would be considered construction.
  9. Related, the funding overview application requirements document outlines that for milestone #1 (people) you must explain how you will "document this support [of people]." Does documentation here refer to final or progress reporting (proving the involvement of volunteers and leaders and participants) or does it refer to satisfying our own progress toward a milestone (i.e.: getting a work contract in place for a paid volunteer coordinator) so that we can be good stewards of our project?
    Answer: Milestones and timelines are anticipated time you expect to meet the milestone.
  10. Where are the pdf versions of Qualtrics? 
    Answer: There is not a pdf version of Qualtrics.
  11. Where in the Qualtrics application do you upload extra documents? I see the upload space for the budget, but nothing for additional documents. 
    Answer: There currently is no option to upload extra documents.

  13. Could a garden layout include plots for individuals to garden on their own for a nominal fee?
    Answer: Possibly, as part of the requirement for RHTP funding the award recipient must indicate how the project is sustainable. However, if there is a split between community gardens versus individuals charged for a fee, detailed justification and scope would be required to ensure clarity on applicability to the solicitation and scope of the grant.
  14. Is built-in/in-ground water infrastructure an allowable expense?
    Answer: We need more information to determine if this would involve construction or materially increase the value of the property or useful life or not.  If this is pertaining to running underground sprinklers, it would be permissible if it does not materially increase the value or useful life as a direct cost and is not considered construction.
  15. Are hard-scape paths (e.g. concrete, asphalt) an allowable expense?  
    Answer: Hardscape paths (e.g. concrete, asphalt) are unallowable expenses if they increase the value of the property.
  16. Are supplies for a gardening cooking class (taste testing/cooking utensils) allowable?
    Answer: Yes, if it applies to the scope and application for the specific solicitation.
  17. Is a small shed to store garden equipment allowable? 
    Answer: A small shed to store garden equipment is allowable if it is not placed on a foundation.
  18. Would small tractor and tillage equipment be an allowable expense?

    Answer: We understand the short turnaround and will take that into consideration when scoring applications, we do encourage you to be as concise as possible.
  20. Could a commercial kitchen set up with food processing equipment be allowed to receive funds?  A place near the garden to clean and process produce.
    Answer: Possibly, RHTP funds could be used to purchase equipment, but it must be sustainable and not materially increase the value of the property or useful life as a direct cost. RHTP funds may also be used for minor renovations and retrofitting of existing areas, but cannot be used for construction, cosmetic upgrades or if materially increases the value or useful life as a direct cost. The location of the commercial kitchen would need to be pre-existent and close to or on the same property as the community garden location. 
  21. How was the RHTP definition of urban and rural areas determined?
    Answer: It is aligned with the state’s application for RHTP funding.
  22. What is the RHTP definition of a rural citizen?
    Answer: Any individual who is not living in the cities of Grand Forks, Fargo, West Fargo and Bismarck.
  23. Does the online application limit the number of words or characters for each question?
    Answer: Some fields may be limited. Applicants are encouraged to be as concise and specific as possible in their responses.
  24. Are garden shed for storing tools and hoses considered buildings.

  1. Is there a recommended type of step tracker or total cost limit per step tracker? i.e. are you thinking of simple pedometers, trackers that include health biometrics, trackers that are interoperable with EHRs or other applications?
    Answer: There are no recommended trackers. However, you want to make sure that the purchase of the trackers fit within the scope of the program. For example, simple trackers will often more than suffice to get the job done for a straightforward eight plus week walking program. Our best advice is to purchase the most affordable ones that will get the job done. Also, be sure to think about providing physical walking logs so that these numbers can be recorded.
     
  2. The Turtle Mountain Band of Chippewa Indians (TMBCI), Belcourt, ND would like to know if a proposal, which reflects the cultural heritage of the tribe will be considered. Specifically, can the proposal include the use of horses in the community walk to align with the cultural connection of TMBCI to their horse relatives? Further, are walks, which reconnect tribal members to the traditional medicines of the Anishinabe, reflect the cultural goals of this health initiative? Answer: Yes.
     
  3. Does every participant need to be provided with an activity tracker? What if they already have something that does this like a phone app, watch Fitbit etc. Do you just need to offer it for each participant, but not all need to accept?
    Answer: No, every participant does not need to be provided with a tracker. And certainly, they can use their own. However, we want to make sure that their numbers get recorded in some sort of step log so that we can measure progress.
     
  4. For health coaching, is there a targeted number or percentage of participants this should be used by?
    Answer: No, there is no specific target. However, we just want to make sure that if people want a little extra attention or guidance program champions are available to help.
     
  5. Is there a matching funding requirement? 
    Answer: No
     
  6. Is there a particular curriculum or walking program that you want us to initiate? I know you've mentioned the Macks Creek, MO multi-week walking program; however, what did their program entail? Was it a six-week program and additional walking opportunities in the community? Or do we tailor this walking program to what seems best fit for our community?
    Answer: There is no specific recommended program. However, whatever the format, it is important that people commit to walking during the week, their numbers get recorded and we strongly recommend having a community walk at least once per week where everyone can gather and move as a community.
    Also, during the onboarding process for grant recipients, we will discuss a variety of different approaches. For the purposes of your application, you should set forth some ideas that you feel will work best for your community. Be creative.
     
  7. How many pages is the narrative allowed?
    Answer: This section will accommodate any answers you might have. However, it is strongly encouraged to be direct and to the point.
     
  8. Will there be a place in the Qualtrics portal to upload a logic model, timeline, evaluation plan?
    Answer: No.
     
  9. Is the $125K award a hard stop or will proposals with modest expansion of the budget target be considered based upon rural community need described in the narrative?
    Answer: We will indeed consider proposals that exceed $125K. However, if more funds are requested, the justification and approaches must be clear and compelling.
     
  10. Should an organization preparing a proposal for this competition create a five-year budget with each year mapping out progress toward the seven recommended outcomes? Or is the budget for year one only allowed and potential funding renewal (year two) remains contingent on how well the project met its goals in that funding year (August 1, 2026 - September 30, 2027)?
    Answer: No, this budget is only for year one. Each year, projects will have the opportunity to renew their funding for up to five years.
     
  11. Would purchase of a health monitoring kiosk for height, weight, bp be an allowable expense for this program?
    Answer: Yes, this could be an allowable expense for this program.
     
  12. Could these grants be used to improve walking facilities, i.e., a gym floor used for the current walking group needs repair?
    Answer: Modest renovations would be allowed. This will be considered and reviewed on a case-by-case basis. RHTP program does not allow funds to be used for construction or cosmetic upgrades, but does allow for minor renovations and retrofitting of existing areas so long as it would not materially increase the value of the property or useful life as a direct cost.

  1. Are for-profit behavioral health providers eligible to apply under the "other healthcare provider" listed on page 4 of the Solicitation Notice?
    Answer: Yes, for-profit providers are eligible for subrecipient work. However, they cannot act in a purchase of service or contractor capacity to be a direct awardee.
     
  2. Can you share examples of supplantation, what counts and what doesn't?
    Answer: Supplanting includes using funds to replace existing state, local, tribal or private funding of infrastructure or services, such as staff salaries. Please see #45 and #48 on pages 46 and 47 of CMS’s FAQs.
     
  3. Is there a list of organizations applying which could use the CHADIS screening, documentation, decision support, patient education and referral system for primary care or mental health and to integrate the two?
    Answer: There is not a list of organizations applying which could use CHADIS.
     
  4. Are personnel costs allowable for program delivery staff, such as counselors, instructors or graduate students who would help implement the intervention and support participants? If so, are there any limits or specific requirements for budgeting these roles?
    Answer: Salaries would be allowable if aligned with the scope of work and the approved application. Administrative costs, which can include salaries, are limited to 10% of the approved modified total direct costs of the budget submitted.
     
  5. Are you basing any funding decisions on existing programs to give an idea of how we can align our proposals to this funding opportunity?
    Answer: The scoring tool, which is used to make funding decisions, is linked in the funding opportunity. 
     
  6. Is this funding only to be utilized for populations that do not already have an identified need? For example, would an established substance abuse treatment center be an eligible applicant to expand services, or no?

    Answer: No. Page 2 of the funding opportunity states: “North Dakota Health and Human Services (ND HHS) is seeking creative evidence-based behavioral health promotion proposals with a focus on promoting mental health, preventing suicide and/or preventing substance use/misuse.  Promotion and prevention strategies are designed for individuals and populations not identified to be in need of treatment for behavioral health conditions.”  There are other ND rural health funding opportunities posted on the HHS website and additional funding opportunities will be posted in the future, and there may be something there that is more aligned to addressing this need. Organizations can apply for multiple funding opportunities.
