
Pathway 1: Training + Internship
Complete a CHW training program that covers the following core skills followed by a 200-hour internship:
- Roles, advocacy, and outreach;
- Organization and resources;
- Teaching and capacity building;
- Legal and ethical responsibilities and conduct;
- Coordination and documentation;
- Communication and cultural competency; and
- Health promotion
The internship provides hands-on, supervised experience to put these skills into practice.

Pathway 2: Community Health Representative (CHR) Training
Individuals who have completed Indian Health Service (IHS) Community Health Representative training may qualify. This training prepares CHRs to deliver community-based, medically guided healthcare, often incorporating traditional Native concepts and practices alongside modern health approaches.

Pathway 3: Supervised Work Experience
Applicants with significant on-the-job experience as a CHW may also qualify.
Requirements include:
- At least 1,000 hours of supervised CHW work within the past three years
- A letter of recommendation from the supervisor confirming the applicant's knowledge, skills, and performance in the CHW role
This pathway recognizes individuals who have learned the work through direct practice and mentorship.

Pathway 4: Reciprocity with Another State
CHWs who are already certified or licensed in another state may apply for reciprocal certification in North Dakota if:
- Their certification/license is current, valid, and in good standing
- The requirements in their state are substantially similar to those in North Dakota
- The Department receives official verification of certification/licensure from the other state
This pathway helps experienced CHWs move seamlessly into practice in North Dakota.

Recertification
CHW certifications expire on December 31st of even-numbered years. To recertify, CHWs must complete 12 hours of continuing education approved by the Department of Health and Human Services. The education must focus on the following CHW competencies:
- Roles, advocacy, and outreach;
- Organization and resources;
- Teaching and capacity building;
- Legal and ethical responsibilities and conduct;
- Coordination and documentation;
- Communication and cultural competency; and
- Health promotion
At least two of the 12 required hours must be dedicated to continuing education in legal and ethical responsibilities.
