About PACE
North Dakota Health and Human Services offers the Program of All-Inclusive Care for the Elderly (PACE).
PACE is a program for people who are 55 years of age and older. Your patient may be eligible for PACE if they live in a PACE service area, would otherwise need a nursing home level of care, and can live safely in their community at the time of enrollment.
With PACE, your patient can access the health care they need while staying in their home and community rather than an assisted living facility or nursing home. Your patient, their family, and you get the flexibility to meet their health care needs with patient-centered services and coordinated care.
This webpage supports our providers. PACE members should go to the Welcome to PACE webpage.
Helpful Information
I want to become a PACE provider
PACE participants have a team made up of health care professionals. The PACE team gets to know them, their living situation, and their preferences to help them make decisions about their care. The PACE team assesses their needs, develops and updates a care plan, and sets goals with the participant. PACE provides all the care and services covered by Medicare and Medicaid, as approved by the PACE team. PACE provides coverage for prescription drugs, medical care, transportation, home care, checkups, hospital visits, and nursing home stays, when necessary.
ND Medicaid oversees the PACE program in North Dakota to ensure compliance with federal and state regulations. We provide state funding for PACE organizations. Northland PACE is currently our only PACE organization.
If you provide a service that you feel could benefit PACE participants, consider becoming a PACE provider. You can become a PACE provider by contacting Northland PACE. Northland PACE contracts with many different providers in the four service areas in North Dakota.
