Supplemental Nutrition Assistance Program (SNAP)

EBT card

If you lost your EBT card, you can request a replacement by calling ebtEDGE at (800) 630-4655. 

Food loss

If you lost food purchased with SNAP benefits you can request a replacement of benefits. You must report the loss within 10 days of when the food was destroyed. You can report it by phone or in writing to the Customer Support Center or a human service zone office

After you report the loss, you have 10 more days to turn in the completed Replacement Request Form (SFN 270).


Temporary Assistance for Needy Families (TANF)

If your TANF Way2Go card is lost or stolen you should report it immediately by calling the Way2Go Card® Debit MasterCard® Customer Service Helpline at 1-844-893-3118 or sending a letter to: Customer Account Services P.O. Box 245997 San Antonio, TX 78224-5997. 


Medicaid

Medicaid ID card and Medicaid ID card for prescriptions

If you lost your Medicaid ID card and/or Medicaid ID card for prescriptions, you can request a new cards by calling the Customer Support Center.

Medicaid Expansion Blue Cross Blue Shield Cards

If you lost your Medicaid Expansion Blue Cross Blue Shield card, you can call Blue Cross Blue Shield of North Dakota at (833) 777-5779 to request a replacement.

Please make sure your mailing address is up to date before requesting a replacement. You can update your address by calling the Customer Support Center.
